Health insurance over fifty — what are you actually paying?

I am 54 and have been coasting on travel insurance, which I now understand is not a plan. Getting quotes between 90,000 and 180,000 baht a year for proper cover with inpatient limits that would survive a heart attack at a Bangkok private hospital. That range is so wide I assume I am comparing the wrong things. What are people over fifty actually holding, what is excluded, and did anyone get caught out by a pre-existing condition clause?
